Yes it does run in the background but it does not consume any extra battery. It only does anything when the Android system notifies it that the network has changed. Stopping it from running in background will make it not work properly and possibly cost you in roaming charges. The notification will only appear when the phone switches from normal to roaming or vice-versa. 
Opening the app would tell you if its running or not.</description>

When I stop monitoring, H+ and two little arrows come up at the top of the screen or 3G but they change constantly. When monitoring is on, nothing is at the top of the screen and data is contantly blocked.I hope this makes sense.</description>

Is there any way to see where I should be within the 3 towers. I feel totally ripped off by 3. My first bill was $250 when it should have been the $69 cap. Cap my ass!!!</description>
